Regulation respecting the assigning of a Social Insurance Number
Act respecting the Québec Pension Plan
(chapter R-9, s. 219).
1. In this Regulation,
(a)  “card” means a Social Insurance Number card;
(b)  “Act” means the Act respecting the Québec Pension Plan (chapter R-9).

2. Every individual who is required by the Act to apply for a Social Insurance Number shall, on the prescribed form, furnish the Régie des rentes du Québec (Board) with the following information:
(a)  his name and given names;
(b)  his name at birth, if it is different from his present name;
(c)  his date of birth;
(d)  his place of birth;
(e)  his mother’s surname at her birth;
(f)  the first given name of his father; and
(g)  any other information which is required in the prescribed form.

3. Where the holder of a card changes his name by reason of marriage or otherwise and is required by the Act to apply for a new card, he shall furnish the Board, on the prescribed form, with all the information mentioned in section 2 of this Regulation, as well as with his Social Insurance Number and with his name and given names prior to the change.

4. Every individual whose card is lost or has been destroyed and who, if he had not already been assigned a Social Insurance Number, would be required by the act to apply for a Social Insurance Number, shall apply to the Board for a new card within 30 days after he learns of such loss or destruction. He is then required to furnish the information stipulated under section 2 and his Social Insurance Number. If it is unknown to him, he is required to make a declaration to the effect that he already had a Social Insurance Number.

5. A Social Insurance Number is held to have been assigned to an individual if such a number has been assigned to the individual by the Canada Employment and Immigration Commission.
The card issued to an individual by the Canada Employment and Immigration Commission is held to have been issued by the Board.

6. The prescribed forms shall be available at all offices of the Board.

8. Where an employee fails to produce his card to his employer within the delay specified in section 197 of the Act, the employer shall file a report with the Board if he has not already filed a report with the Canada Employment and Immigration Commission, giving all details necessary for the identification of the employee.
If the employee produces his card to the employer after the filing of the report, the employer shall promptly so notify the Board.
