21. A candidate who wishes to take a supplemental examination must send the registrar, at least 60 days prior to the examination date set by the board of directors, in accordance with section 8:

1°  the duly completed supplemental examination registration form appearing in Schedule II;

2°  a certified cheque or postal money order covering the fee set by the board of directors, that fee being non-refundable.
