9. Any petition for certification shall be accompanied by a certified true copy of the resolution provided for in section 25 of the Labour Code (chapter C-27) and shall contain the following information:
a) the name of the petitioning association and, if applicable, the body to which it is affiliated;
b) a description of the bargaining unit sought;
c) the employer’s name and the address of the establishment(s) concerned.
