PROFESSIONAL INSPECTION COMMITTEE OF THE ORDRE DES CHIROPRATICIENS DU QUÉBEC

STATEMENT OF FACTS

I, the undersigned, ______________________________, secretary of the professional inspection committee of the Ordre des chiropraticiens du Québec, hereby declare that:
