13. A contact lens listed in Part I of Schedule I is insured in respect of a person with a visual deficiency who is 6 years of age or over only if that person has one of the following deficiencies:1° an antimetropia or anisometropia with a difference of at least 2 diopters between the 2 eyes;
2° a myopia of at least 5 diopters;
3° a hypermetropia of at least 5 diopters;
4° a regular astigmatism with a difference of at least 3 diopters between the major meridians;
5° an ocular pathology that has been medically diagnosed and that requires the wearing of therapeutic contact lenses upon prescription from a physician.
