576. The court assigns to the adopted person the surname and given names chosen by the adopter unless, at the request of the adopter or of the adopted person, it allows him to keep his original surname and given names or assigns him a surname consisting of not more than two parts taken from those forming the adopter’s surname or the surnames of the child’s father and mother or parents with whom a pre-existing bond of filiation has been recognized.
