Section 16.1
Disposition versions
16.1. At the request of the Commission, a physiotherapist, a physiotherapy technologist or an occupational therapist must provide a report whose form and content must comply with the form in Schedule III.1 or, if sent using another technological medium, complying with that authorized by the Commission.
The report form is available on the Commission’s website.
The report must be sent to the Commission and to the health professional in charge of the worker within 15 days following the date of the Commission’s request.
16.1.
