129. The application for review must be made on the form prescribed by regulation under paragraph 2 of section 263, otherwise it is deemed not to have been filed.

129. On pain of being dismissed, complaints must be made on a complaint form prescribed by regulation under paragraph 2 of section 263.

129. At the demand of the complainant, the clerk of the municipal corporation shall furnish him with a complaint form prescribed by regulation under paragraph 2 of section 263.
That form must include a conspicuous note that its use is not obligatory provided that the allegations of the complaint comply with section 128.
