Versions
10. An application for registration of a plan must be made on the prescribed form and contain the following information:
(a) the identification of the employer;
(b) the nature of the business;
(c) the name of the plan;
(d) the statistics concerning membership in the plan;
(e) the names and addresses of the administrators;
(f) the funding medium.
