Regulation respecting the standards for the form and content of verbal or written prescriptions made by a dentist
Dental Act
(chapter D-3, s. 19, par. c).
DIVISION I
WRITTEN PRESCRIPTIONS
1. A dentist shall not furnish prescription slips for medication, devices or treatments unless they clearly indicate:
(1)  his name, address and telephone number;
(2)  the date of the prescription;
(3)  the patient’s identity including his address;
(4)  his signature;
(5)  his licence number;
(6)  in the case of a treatment, the nature of the treatment;
(7)  in the case of a medication, the name of the medication, the pharmaceutical form, the concentration, the quantity prescribed or the length of treatment, the dosage and the number of authorized renewals;
(8)  in the case of a device, the kind and description of the device with, in the case of a removable prosthesis, a diagram of the device;
(9)  for a renewable prescription, its period of validity.
Subparagraphs 5 and 8 do not apply to prescriptions written at a hospital centre for a user to whom the Act respecting health services and social services (chapter S-4.2) applies or for a beneficiary within the meaning of the Act respecting health services and social services for Cree Native persons (chapter S-5).

2. A dentist who makes a verbal prescription for medication or treatments shall clearly indicate the items set out in section 1.
